ELECTRONIC LETTER OF AGENCY AUTHORIZING CHANGE IN CUSTOMER'S NATURAL GAS SERVICE PROVIDER TO Stream ENERGY GEORGIA, LLC ("Stream ENERGY"), A CERTIFIED NATURAL GAS SUPPLIER IN GEORGIA

By this Letter of Agency, the Customer ("you") hereby affirm the following: 

You are the utility account holder or a person that has legal authorization on this account and confirm that the billing name, billing address, and service address that you provided is correct:

The full terms, conditions, and nature of the service to be provided to you by Stream Energy have been clearly disclosed to you in the Terms and Conditions and Disclosure Statement, of which you acknowledge receipt.

You are the billing customer of record as identified above, and electronic acceptance of this Letter of Agency in the enrollment process will serve as the signed authorization of this Letter of Agency as of the date of acceptance.

This information may be used by us to determine whether it will commence and/or continue to provide energy supply service to Customer and will not be disclosed to a third party unless required by Term and any Renewal Term. Customer may rescind this authorization at any time by providing written notice to Stream or by calling Stream at 1-866-447-8732. Stream reserves law. Customer’s execution of this Agreement shall constitute authorization for the release of information Stream. This authorization will remain in effect during the Initial the right to cancel this Agreement in the event Customer rescinds authorization. Energy supply will continue to be provided by the ESCO and energy delivery shall continue to be provided by customer’s utility. Said utility will be also available to respond to leaks or other emergencies should they occur. Customer authorizes Stream to obtain and review information regarding Customer’s credit history from credit reporting agencies and the following information from the LDU: consumption history, billing determinants, utility account number, credit information, public assistance status, existence of medical emergencies, status as to whether the customer has a medical emergency, is human needs, elderly, blind or disabled and data applicable to cold weather periods under PSL § 32(3) and information pertaining to PSL § 33, tax status and eligibility for economic development or other incentives. This information may be used by us to determine whether it will commence and/or continue to provide energy supply service to Customer and will not be disclosed to a third party unless required by Term and any Renewal Term. Customer may rescind this authorization at any time by providing written notice to Stream or by calling Stream at 1-866-447-8732. (3)Stream reserves law. Customer’s execution of this Agreement shall constitute authorization for the release of information Stream. This authorization will remain in effect during the Initial the right to cancel this Agreement in the event Customer rescinds authorization. Energy supply will continue to be provided by the ESCO and energy delivery shall continue to be provided by customer’s utility. Said utility will be also available to respond to leaks or other emergencies should they occur. I have read and understand this Authorization and the terms of service that describe the service(s) I will be receiving. I also acknowledge that my account documents, notices or other disclosures will be delivered electronically to the email address I provided, unless the company is required to send by mail or another format. If email does not work, the company will attempt to deliver account documents by mail. I agree to notify the company if my email address changes to ensure that my documents can be delivered. I represent that I am not currently approved for or enrolled in Ohio’s Percentage of Income Payment Program (PIPP) or any such utility program. If I become approved for PIPP or my utility’s arrearage crediting program while taking service from Stream under this Agreement, I promise to notify Stream in writing in order to be switched to my local utility’s standard offer service after the next meter read date.

This Agreement will automatically renew on the expiration of the estimated billing cycle for the initial term of the contract. The exact date that your initial agreement will begin and end, depends on when your local utility company accepts our request to enroll you as our supply customer. This process can take up to forty-five (45) days, but usually less. Your energy supply service will not be interrupted during this time. Your renewal contract term will begin on the following billing cycle. You will be provided with a Notice of Automatic Contract Renewal not more than 60 days and not less than 30 days before the expiration of this Agreement. That notice will advise you of product options that are available to you and the price that you will be automatically renewed to if you do not call us by the date specified in that notice to select a different product option or to reject automatic contract renewal. That Notice will also advise you of the procedure for notifying us if you want to reject contract renewal and terminate your existing contract at the end of its term.

I understand participation in the Low-Income Home Energy Program or the Percentage of Income Payment Plan (PIPP) may affect my eligibility to take service from a competitive retail electric service provider. I represent that I have not received assistance from the Low-Income Home Energy Program in the past 12 months and that I am not currently approved for or enrolled in PIPP.
